INSURANCE
I am an in-network provider with BCBS PPO and HMO for individual sessions only. I will verify your benefit coverage prior to us meeting, and will need the following information to do so: customer service number on the back of your card and your member and group IDs. Please note that benefits are an agreement between you and your insurance company. I cannot guarantee any insurance coverage or reimbursement. If you have eligibility concerns or questions, please contact your insurance provider directly.
I will be required to give and submit a clinical diagnosis to your insurance provider. The insurance provider will determine if the diagnosis meets medical necessity, which is required for services to be covered.
In the case where I am out of network, a superbill can be provided weekly/monthly for clients to use for insurance reimbursement issued themselves.
